What this policy covers

This policy covers bookadoc2u.com — this website only. It explains what this website collects and what happens to it. Care you receive from BookaDoc2U as a patient is covered separately by the Notice of Privacy Practices you are given when you register as a patient, which is the document that governs your protected health information under HIPAA.

What this website collects

This website collects information in one place only: the appointment request form. That form asks for

• your first and last name,
• your email address,
• a phone number,
• the type of visit you are asking about, and
• a preferred date, if you choose to give one.

That is the complete list. The website does not ask for, and the form has no field for, your medical history, symptoms, diagnoses, medications, insurance details or payment details.

Please do not send medical details through this website

The appointment form is deliberately limited to scheduling. Ordinary email and web forms are not a secure channel for health information. When we call you back to confirm your appointment, we will take your medical history through a channel appropriate for it. If you send clinical details anyway — for example inside your name field — we will handle them under the same confidentiality obligations that apply to the rest of our practice, but we ask that you do not.

What we do with it

An appointment request is sent to our practice inbox so a member of our team can call you back. We use it to contact you about the appointment you asked for. We do not sell it, we do not rent it, and we do not use it for advertising.

Where it is stored

This website has no database and stores nothing itself. Your request is delivered to our practice inbox and is retained there under our practice's records policy. Our email is operated on infrastructure run by Agyeman Enterprises rather than a third-party marketing platform.

Cookies and tracking

This website sets no cookies. It runs no analytics, no advertising pixels, and no third-party trackers. Nothing on this site profiles you or follows you to other websites. Our web server keeps ordinary short-lived technical logs, which include IP addresses, to keep the site available and to limit automated abuse of the appointment form.

Your choices

You can ask us what appointment-request information we hold about you, ask us to correct it, or ask us to delete it. Email office@bookadoc2u.com or call 415-569-6263. Rights over your medical record as a patient are described in the Notice of Privacy Practices you receive at registration.

Children

This website is not directed at children, and the appointment form is intended to be completed by an adult — either for themselves or on behalf of someone in their care.

Changes

If this policy changes, the date at the top of this page changes with it.
